Report PD & PA changes

This form is intended for our PGME programs, Departments and Divisions to:

  • Report a new, departing or change of access for a:
    • Program Administrator
    • Program Director
    • Site Director
    • Departmental Administrator
  • Enable swift access to, or removal from, PGME and external systems, and communication lists.
  • Note: This form should also be submitted in cases where staff or faculty are going on an extended leave to ensure system(s) access is removed and reinstated as appropriate. Please submit two forms, one to remove access when the individual departs and another to reinstate access when their return date is known.
